Description:
Assisting with the scheduling of care
- Help to co-ordinate delivery of Home Care Assistant work schedules and the duty desk rota each week.
- Update details of new service users onto the Tagtronics database and schedule visits.
- Assist the manager in booking employee supervisions, appraisals, etc.
- Arrange appropriate shadowing for new Home Care Assistants.
- Schedule new service users’ first visits with the Risk Assessment Officer.
- Monitor any problems with staff and report them to HR.
Duty desk
- Answer phones and maintain a friendly and professional relationship with service users and Home Care Assistants.
- Respond to queries promptly.
- Update notes and queries regarding service users on the database.
- Review call monitoring reports throughout the working day and investigate late or missed visits.
- Take referrals of new care packages and pass them to the Operations Manager to assess their suitability.
Maintenance of the Tagtronics database and service user files
- Assist with inputting new service user information and care plans onto the Tagtronics database.
- Write up care plans for new service users and complete all new service user packs.
- Write up service user initial reviews and file them appropriately.
- Update any changes to the care plan and ensure this is signed by the service user.
- Update service user files in accordance with CQC regulations and standards.
- Prepare the assessment paperwork for service user reviews.
Care work
- A Care Co-ordinator will be given full training as a Home Care Assistant and will carry out care visits when required.
- Keep care training up to date; learning and acting on all current advice; completing required tests.
