Remuneration:£500 per day for 30 UDAs
Description:Locum Performer Dentist required for 2-5 days per week (MON, WED, THUR, FRI) in a busy, mixed, well established, INDEPENDENTLY OWNED practice near Southampton, Hampshire.
• 4000 UDAs available
• £500 per day for 30 UDAs
• Existing, well maintained patient list
• Locum to Permanent option
• On-going till place permanent Dentist
• Fully computerised surgeries
• Den Plan and Invisalign experience preferred
• Digital x-rays
• Rotary endo
• Full support team
• VTE then will also be paid £500 per month extra
• Implantologist on-site
• Any special interests welcome - particularly Invisalign, Facial Aesthetics and Implant Restoration
• Free on-road parking
• Available now

Compliance requirements:
- Full NHS Performer Number
- Local Area Team confirmation
- GDC certificate
- UK Blood test report showing Hep B immunisation and titre levels, Hep C, HIV and TB results dated within last 5 years
- Indemnity certificate
- Passport
- Enhanced DBS (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
- Degree certificate
- CPR/AED/Medical Emergencies certificate dated within the last 12 months
